[sharechat] Bruce Sheppard as proxy?

Hi fellow chatters,

I wanted to suggest to someone that they appoint Bruce Sheppard of 
the Small Shareholders Association as their proxy at an upcoming 
shareholders meeting.  Most proxy forms are worded something like.

I appoint .......................
of ..............................
as proxy

I can tell them what to put in the first line but not the second one. 
Anyone have a postal contact address for Bruce Sheppard?  TIA
